The fine print – this survey was built using the Formidable Forms plugin for WordPress. We have changed the settings to not collect IP addresses along with the form submissions. We are not requiring users to provide personal data, such as name or address, for survey submission. Survey responses will be deleted from the local WP database after the analysis of the results is complete.
